This form is to help us evaluate our program. Please fill it out and return it to your parent representative. Please let us know how you think you did during the season. This is your opportunity to have your views on the record. You may also review each of the player evaluations and comment in reply to concerns raised.
This form is to help us evaluate our hockey program. Please fill it out (in consultation with your child) and return it to your parent representative. The parent representative then provides the form to the Community Centre Coach and Development Representative or President who then provides it to the Director of Coach and Development for LSMHA. All informations may be reviewed by the coach upon request who will be able to reply to specific concerns.
